Creach Consulting Group partnered with IHN-CCO over several years to provide technical assistance and training to primary care practices across its network. This work supported practices in advancing behavioral health integration, complex care management, addressing health-related social needs, and meeting medical home standards of care.
In addition, Creach Consulting Group conducted a value-based payment assessment of the primary care network and developed recommendations designed to support providers while improving health outcomes and managing total cost of care.
